TATTOO RULES / BOOKING FORM
I understand that I MUST BE at least 18 years of age to be tattooed. 

I MUST present a valid photo ID including date of birth, name and photo on the day of my appointment. 

I understand that I must give a minimum of 2 business days notice to cancel or re-schedule my appointment. Our business days are Tuesday through Saturday, plus all statutory holidays.

I must arrive at least 5 minutes before my scheduled appointment. 

I understand that I CAN NOT consume any alcoholic beverages 48 hours prior to my appointment. 

I MUST have something to eat just before my appointment or no longer than 2 hours before my appointment (excluding fast food, power or chocolate bars, or fruits or vegetables eaten without heavier food such as protein). 

I MUST be in good health (meaning free of influenza, common cold or other illnesses including infections, abscesses, vaccination side effects, etc). For my own safety I know I need to notify my tattooist of any medication I may be on.

I MUST notify New Dimension Tattoos of any surgery I have had in the last 12 months or of any upcoming surgery I may be going for.

I CAN NOT be on any antibiotics. I need a good sleep the night before. 

I MUST avoid tanning for minimum of 7 - 10 days before my appointment including spray on tanning.

I understand that all deposits are non-refundable & non-transferable from design to design or from person to person. My deposit will be deducted from the last session of my tattoo project. I understand that I will lose my deposit if I decide to change my tattoo idea after it has been drawn up. New deposit will be required for every new drawing. 

I understand that I will lose my deposit if I change my mind on a design after it has been drawn, or if I do not show up for my appointment, or if I do not approve the final version of my drawing in time which is 48 hours prior to my appointment. In the case of a late artwork approval my appointment time may need to be cancelled and/or rescheduled and the loss of my deposit will be the result of it. 

After a deposit has been lost, a new deposit will be required before scheduling a new appointment. 

My deposit will automatically expire 12 months from the last date of activity, whether it be an initial date of leaving my deposit or my last tattoo session. All deposits rolled to following session are subject to same policies.

NO children under 12 years old are allowed in tattoo area or unattended in the waiting room.

For women only: I understand that I cannot be tattooed if I am at any stage of pregnancy or breastfeeding.

I understand that failure to full-fill these requirements will result in loss of my appointment as well as my deposit. I understand that any design changes made after my arrival for my appointment may result in loss of my deposit and possibly my appointment depending on artist’s time availability.

IMPORTANT NOTE: To insure your complete safety and quality of work to be performed, please let us know whether you suffer from any major illness including epilepsy, Parkinson’s, seizures, serious heart problems, hemophilia, iron deficiency or any other illness.
